Assisted Living Referral Services
Embrace Senior Advisors assist clients in finding long-term care options using a system that offers multiple living options considering the five facets of living: care, cost, location, social preferences, and spiritual needs.
After an initial conversation, our advisors will typically make an appointment to meet with you in person, or if you are not able to meet in person, then we can offer a virtual Zoom call. We can gather information from your medical team, social workers, family, and other professionals involved in your plan of care. In limited circumstances, information would be gathered from you without a face-to-face meeting but conducted through email with written consent.
In case of an immediate move, where a decision must be made right away for your peace of mind and your loved ones’ safety, assessments can be done in other ways to meet this special request.
Once we have gathered all the information, we provide care recommendations about which senior living communities best fit your care needs and budget best. We personally accompany you to tour the care communities you select, and once you’ve made a decision, we coordinate with their staff to arrange for medical evaluations and any other requirements for acceptance. If there are additional considerations, such as a need for specialized medical equipment, we can also help coordinate with social workers and medical providers to make sure everything is in place before you or your loved one arrives home.
Throughout the entire process, our goal is to provide timely and comprehensive information, thoughtful support, and consideration of all of your needs.
Referral Fees
- EMBRACE collects a referral fee from contracted long-term care providers/communities after a client moves in.
- Embrace has no ownership in or bias toward any long-term care community.
- EMBRACE refers to contracted and non-contracted long-term care communities.
- Contracts with long-term care providers for your referral are good for one year from the date of our contact with the long-term care provider.
- You do not pay EMBRACE for the Length of the Contract.
- This agreement will remain in effect until the end of one year unless you and the client have decided to terminate.
